Pre Production Planning; Risk Assessment

I have removed the exact location and my mobile number for safety reasons but the original is completely filled in

I was unaware of risk assessment forms before I started to film my music video but I soon realised that it is essential to do a location risk assessment and the information you collect is very important for the safety of you and any other participants. 
If I decide to use a second location I know that I need to complete a new risk assessment form.
